Age Friendly Health Systems

Age-Friendly Health Systems is an initiative of The John A. Hartford Foundation and the Institute for Healthcare Improvement (IHI), in partnership with the American Hospital Association (AHA) and the Catholic Health Association of the United States (CHA), designed to meet this challenge head on.

What is an Age-Friendly Health System?

An organization that reliably provides evidence based elements of high-quality care for all older adults served by a system: What Matters, Medication, Mentation, and Mobility.

We invite you to join our new Colorado State AFHS Coalition, in partnership with the Alzheimer's Association and Telligen QIN-QIO. Through this coalition you will become part of a larger network of Age Friendly Health Systems (AFHS) throughout the state of Colorado. The vision of the coalition is to promote and support the adoption of the Institute for Healthcare Improvement’s (IHI) AFHS framework, concepts and components as the standard practice of care across all Colorado healthcare settings.

Through the Geriatric Workforce Enhancement Program (GWEP), we can assist your team in the journey to become a certified Age Friendly Health System (AFHS)!

How can the Center help in this process?

    • Assist in the AFHS submission process
    • Provide ongoing team coaching as your organization works through the change process
    • Provide access to the AFHS Sphere to assess your team's readiness for change and monitor successes along the way
    • Connect you with peer networks related to the AFHS process
